i(1.)         Name of the candidate as per High-school / College certificate(s) by approved Board/University. The application form should be filled by his / her own hand writing with Blue Ink only.

i(2.)         Fathers Name / Guardians Name: as per High-School / University Certificate(s) [same as i(1.)].

i(3.)         Date of Birth as per High School Certificate(s)/ Birth Certificate if available.

i(4.)         Nationality/ Religion: as per birth / immigration what-so-ever e.g. Indian (and/or) in case of Dual Nationality plz. Mention in detail with passport number and type of nationality.

i(5.)         Marital Status: Married / Single / Other (this field is optional in exceptional case).

i(6.)         Permanent Address: i.e. your own house / or / parental house (not rented / pg etc.) if you do not have permanent address plz give 2 reference of Govt. Employees (in India only) along with phone number.

                Present Address: i.e. your present / temporary / postal - address if any (write “—same—“ in case same as Permanent Address.

i(7.)         Phone Number(s): please give alternative contact number(s) if applying for correspondence course.

i(8.)         Medium of Examination: the course / examination is available in Hindi / English only (You need to hire personal translator in your country if applying from out side-India.

i(9)          Qualification: candidate should fill all academic / medical / professional / technical / other qualification as applicable.

i(10.)       Physical / Health Status: this field is compulsory for Normal / Disable / Physically Challenged / other case.

i(11/12.) For Medical Practitioner(s): the field is necessary for medical professional(s).

i(13.)       In this section candidate can mention any –thing special for his / her case e.g. additional books, sample papers if required, special notes if required, any request if applicable.

i(14.)       This field is necessary for all candidates.

i(15.)       Hostel / PG / Accommodation Facility: availability of Hostel / PG / Accommodation is an Optional Field, Accommodation or Hostel facility is responsibility of the student only, we only provide voluntary support to outstation (out of Delhi / NCR) students if they unable to find accommodation their-self.

(b)           Passport Size Photo: 1+3 passport size photo(s) alongwith application (1 on form + 3 spare) please only first photo on the form will be self attested (i.e. by the applicant).
